What Is Chronic Care Management?
Chronic Care Management is a Medicare-supported program for patients living with two or more chronic conditions, such as:
Diabetes
High blood pressure
Heart disease
Arthritis
Asthma or COPD
Depression
And many others
Our goal is to help you stay healthy and avoid unnecessary hospital visits by providing ongoing care coordination and support outside your regular appointments.
What You Can Expect
As part of our CCM program, you’ll receive:
A dedicated care coordinator who works closely with your provider
Monthly check-ins to review your progress and make sure your care plan is on track
Help with scheduling appointments, refilling medications, and managing referrals
Ongoing education about your conditions and how to manage them
Medication reviews to help you understand what you're taking and why
Coordination with specialists and other healthcare providers to keep your care connected
