This isn’t a physical exam — it’s a conversation about your overall health and future.

What Is an Annual Wellness Visit?

An Annual Wellness Visit is a once-a-year appointment covered by Medicare Part B. It’s designed to create or update your personalized prevention plan, based on your current health, lifestyle, and risk factors.

This visit helps your care team identify potential health issues early, recommend screenings, and develop a plan to help you live a longer, healthier life.

What’s Included in an AWV?

During your Annual Wellness Visit, we will:

Review your medical and family history

Review your current medications and supplements

Record your height, weight, blood pressure, and other key vitals

Assess your cognitive function and memory

Screen for fall risks, depression, and safety concerns

Develop or update your personalized prevention plan

Provide referrals for screenings (like mammograms or colonoscopies), if needed

Discuss advance care planning, if appropriate

You are eligible for an Annual Wellness Visit if:
  • You have had Medicare Part B for more than 12 months

  • You have not received an AWV in the past 12 months

  • New to Medicare? You may qualify for a “Welcome to Medicare” visit within your first 12 months of coverage.
